Fragestellung: Zur Behandlung von Multiple Sklerose (MS)- Tremor mittels Tiefer Hirnstimulation wird als Zielpunkt häufig der Nucleus ventralis intermedius thalami (VIM) gewählt. Anhand von intraoperativen Daten konnten bisher Kohärenzen zwischen Muskelaktivität und lokalen Feldpotentialen (LFP) lokalisiert werden. Die Kausalitätsverhältnisse dieser beiden Signale zueinander sind allerdings derzeit unklar. Methoden: Bei der Implantation von Stimulationselektroden in den VIM wurden intraoperativ simultan LFPs von 5 Makroelektroden (Abstand 2mm) und Oberflächen-EMG-Signale vom M. extensor digitorum communis (EDC) sowie M. flexor digitorum superficialis (FDL) in fünf Kerngebieten (3 Patienten mit MS-Tremor) mit dem INOMED MER-System abgeleitet. Auf verschiedenen Höhen wurde die neuronale LFP- und Oberflächen-EMG-Aktivität für 30–90s aufgezeichnet und offline analysiert. Zuerst wurden die Daten einer visuellen Artefaktkontrolle unterzogen und dann zur Bestimmung der Phasenbeziehung zwischen LFP und EMG Hilbert-transformiert. Anschließend wurde die Phasendifferenz bestimmt. In Zeitfenstern von 20ms wurden signifikant häufiger auftretende Zeitverschiebungen statistisch abgeschätzt. Zur weiteren Bestätigung der Richtungsverhältnisse wurde zusätzlich ein autoregressives Modell angepasst und die Granger-Kausalität bestimmt. Ergebnisse: Zwischen VIM-LFPs und dem Tremor-EMG fanden sich signifikante Phasenverschiebungen. Diese waren zum Großteil efferent (VIM-LFP vor EMG: 69%), teilweise aber auch afferent (EMG vor VIM-LFP: 31%). Signifikante Häufungen der Phasenverschiebungen bei den drei Patienten traten sowohl im Bereich von -110ms bis -90ms als auch im Bereich von -30ms bis -10ms auf, so dass das VIM-LFP mehrheitlich die Muskelaktivität anführt. Die Granger-Kausalität bestätigte die Richtungen dieser Beziehungen. Schlussfolgerungen: Bei MS-Tremor treten mehrheitlich efferente vereinzelt auch afferente Phasen- und Kausalitätsbeziehungen zwischen EMG und LFP's im VIM auf. Die zwei signifikanten efferenten Zeitfenster deuten möglicherweise auf unterschiedliche Vermittlungswege zwischen VIM-LFPs und Tremor-Signal im Muskel hin.
